The application form described above is used for acquiring serpentine pendanthead. In order to get it, pasting three tickets, included in Japanese Clue Books (published by Locus) for Ultima IX and Ultima Collection, and Ultima Collection Japan Edition, on the form and sending it to Locus until May 31, 2000. metal pendant heads were sent the all people who sent the application form. In addition, selected 100 people had received silver pendant heads, along with velvet cases( closed, opened ). Each silver pendant heads has a serial number on the rear side
